Elective Surgery
The Booking Office is located in Level 1 West, Room 8, of the Clinical Services Building and manages the Sydney and Sydney Eye Hospital Elective Surgical Waiting List.
If you have not been seen by a doctor, please refer to the Outpatients Department page by clicking this link.
Do you need surgery?
When you see a doctor either in the Outpatients Department or private rooms the doctor will decide if you need to have surgery.
If you need surgery, your doctor will fill in a Sydney and Sydney Eye Hospital Recommendation for Admission form. On this form the doctor will indicate the waiting time for the surgery.
The maximum wait time is 12 months.
Recommendation for Admission Form
The Recommendation for Admission form needs to be sent to the Booking Office. At this point you are placed on the elective surgical waiting list.
Your waiting period for your surgery starts once the Booking Office receives your Recommendation for Admission form.
You will be sent a letter telling you that you have been placed on the waiting list.
What next?
When it is time for your surgery you will be contacted by the Booking Office and offered a date for surgery.
