The Southcare Geriatricians are an Outpatient Department at Sutherland Hospital comprising of 8 specialists caring for older people in the Sutherland Shire. 

Our team provide a comprehensive assessment of your physical and cognitive condition and provide support and advice for improved health and independent living.

We assist older people by preventing and treating disease and disability that often comes with ageing.

A person with aged care illnesses requiring a medical assessment or Psychogeriatric consult.

  • Cognitive impairment including dementia
  • Osteoporosis
  • Falls and Fractures
  • Incontinence
  • Osteoarthritis
  • Depression
  • Frailty
  • Mobility Issues
  • Very old people with medical problems

Exclusion Criteria: 

People requiring social or functional assessment without the need for a medical Psychogeriatric or general Aged Care Assessment should be referred to Southcare indicating service/s required
 

The process for referring a patient is: 

  1. GP/referrer to send through referral via fax to 9540 7870 or via email on SESLHD-Geris@health.nsw.gov.au
  2. The referral will be checked to ensure it is valid, if not extra information will be sourced from the referrer;
  3. If valid the referral will be triaged by clinic doctor;
  4. Appointment made as per doctor’s recommendation ;
  5. Patient notified by confirmation letter/phone call as well as a call 2-3 days prior to appointment.

A valid referral requires:

  1. Written by a medical practitioner in a private practice or an SMO;
  2. Written to a named Doctor within the clinic;
  3. Signed and dated by referring practitioner and include their Provider Number and place of practice;
  4. Include length of referral, unless it is a 12 month referral;
  5. Patient to have signed financial consent form.
     

On average 6 months for a new patient but all referrals are prioritised and allocated according to urgency.

If you hold a valid Medicare card the visit is Bulk Billed.

If you do not hold a Medicare card a fee will be charged on arrival, please contact the clinic for further information.
