Aboriginal 48 Hour Follow Up Service

This service is for Aboriginal or Torres Strait Islander people living in the South Eastern Sydney Local Health District (SESLHD) who need ongoing care after leaving  hospital. This area is the traditional land of the Dharawal, Gadigal, Wangal, Gweagal and Bidjigal peoples.

Our Team 

You will receive support from a team of Health Care Workers

What we do

After you leave hospital, a staff member will call you within 2 days to have a yarn about:
•    How you’re feeling
•    How you’re managing at home
•    If you understood the doctors’ instructions  
•    Whether you need any extra help  
•    If you’ve made your follow-up doctor appointment
•    If you’ve gotten your prescriptions from the chemist
•    Connecting you with other health workers or services outside the hospital
•    Providing information on services to help you recover

This service is for Aboriginal and Torres Strait Islander people over 15 who have  recently been in hospital.

• Prince of Wales Hospital
• The Sutherland Hospital  
• Sydney and Sydney Eye Hospital
