HealthPathways Program

The SES HealthPathways website aims to improve the management of patients in and between primary and secondary care, through clearly defined pathways and protocols, as well as improving communication between clinicians.

It is available to local health and social care service providers including local General Practitioners, Allied Health Professionals, residential aged care facilities, local government and non-government organisations, and South Eastern Sydney Local Health District clinical staff. Pathway Development

Pathways continue to be developed with the team celebrating 200 live pathways in early May 2020. These include a suite of pathways related to COVID-19 with the current focus on developing various palliative care and mental health pathways.

Pathway priorities are directed by primary care clinicians and  specialist hospital staff. To register your request for pathway development follow the link to our home page (registration required) or contact the project team:
