Respiratory Coordinated Care Program
Community Respiratory Service (RCCCP)
The Respiratory Coordinated Care Program (RCCP) is a hospital-based community program implemented across both St George and Sutherland Hospitals that provides home visits to patients who are unwell from a respiratory condition. This specialised hospital-based community program is also designed to assist people with advanced lung disease to live optimally well in their homes.
Our nurses and physiotherapists work together to provide exceptional, specialised respiratory care, short and longer term, which has proven to decrease the number of visits to hospital and reduce length of stay, allowing patients to remain optimally well at home with the added support of our 7 day per week service.
We are contactable between the hours of 0800-1630, 7 days a week and whilst providing specialised respiratory care, we often act as the link between the hospital and the patient, and/or the patient and their GP.
