Information for Health Professionals - Community Dementia Services
Our dementia nurses can visit your patient at home and give them and their family/carers information and advice on dementia and memory loss. They help them access respite or day centre services.
We accept referrals for persons who are:
- Aged 65 years or older (50 years or older for Aboriginal persons)
- Living at home in the Randwick or Botany Local Government Area, and
- Requiring a functional assessment related to dementia or cognitive impairment.
To arrange this service for your patient and their family follow these steps:
1. Your patient will need an Aged Care Assessment
This is a Government subsidised service. All patients need an Aged Care Assessment. Ask your patients, or their family/carers to arrange this assessment by visiting My Aged Care at: www.myagedcare.gov.au or by phoning 1800 200 422.
If they need support to arrange this assessment, they can call the Care Finder service by calling 1800 346 337 (Monday to Friday 8:30am to 4:30pm).
2. Make a referral to the Northern Network Access and Referral Centre
Email a referral through to NNARC to arrange an appointment for your patient with our Community Dementia Service. Email: seslhd-nnarc@health.nsw.gov.au.
Note: If you are working at Prince of Wales Hospital you can make a referral through our Electronic Medical Records system via a consult order.
