Diabetes High Risk Foot Service - Information for Health Professionals

We care for patients with diabetes-related foot wounds or Charcot Neuroarthropathy. We also help patients with diabetes and at a high risk of getting foot wounds. 

Our services include:

  • Diabetes-related foot wound management
    • debridement
    • wound dressing
    • total contact casting
    • other pressure reducing techniques/devices that prevent foot injury and enhance wound healing
    • investigations such as wound swabs and imaging referrals
  • Charcot’s neuroarthropathy foot immobilisation and temperature monitoring.
  • Foot care for diabetes-related wound and /or infection prevention for people with very high risk foot status*.
  • Walking foot-pressure and gait assessment.
  • Neurovascular assessments including toe-pressure readings.
  • Supporting applications to Enable NSW for medical grade footwear by submitting equipment requests, where appropriate.
  • Knowledge sharing with patients and staff.
  • A multi-disciplinary clinic for patients with diabetes and Charcot’s neuroarthropathy, reoccurring foot wounds and/ or diabetes related foot wounds that are not progressing towards healing with conservative treatment.

*Patients at risk of diabetes-related food wounds must meet the following inclusion criteria:

 Very High risk/ High priority: A confirmed diagnosis of diabetes + any one (1) of the following:

  • Foot wound
  • Healed diabetes related leg, foot or toe amputation
  • Charcot’s neuroarthropathy foot
  • A healed foot ulcer with skin or foot shape changes

High Risk/ Lower priority: A confirmed diagnosis of diabetes + any two (2) of the following:

  • Peripheral arterial disease (PAD)
  • Loss of Sensory perception (neuropathy)
  • End stage renal disease (ESRD)
  • A healed foot wound
  • Callus, or toenail problems.
